CAUSES OF PILES / HEMORROIDS
# Riding:e.g. two wheelers, horse riding.
# Sitting in awkward position like ,prolonged sitting in knee thigh position or squatting position 3-Dietary habits: e.g. Irregular excessive heavy diets.
# Bowel habits: Irregular , over straining at the time of defecation.
# Acute attack of diarrhea & dysentery.
# Last week of pregnancy.After parturition the piles masses become larger and persistent.
# Infective organisms.
# Trauma
SYMPTOMS OF PILES/HEMORRHOIDS
Maximum cases in young & adult persons between 21-40 years of age. So an active sexual life is the causative factor of piles.Few important features of piles are as under-
GASTRIC DISTURBANCES: Like indigestion,constipation,loss of appetite,hyperacidity and abdominal distention.
Bleeding: it is the first symptom of haemorrhoid in the form of drops or stream.Bleeding may occur apart from defecation when pile mass has attained sufficiently larger size and remains prolapsed at anal outlet.The undercloth may be stained with blood discharge at any time.The blood is fresh and brightly red in colour.
PROLAPSE :It is the late symptoms of piles.Initially the pile mass protrudes at anal orifice during act of defecation and slips back into the anal canal after defecation efforts have overcome.Further at later stage the pile mass slips out of the anal canal and does not return back after defecation and needs reduction by digital manipulation.Finally at last stage pile remain permanently prolapsed at anal anal orifice.The anal mucosa remains permanently in contact to underclothes,which are stained with anal discharge.
DISCHARGE:The mucosa of the prolapsed pile mass is exposed externally and secretes mucoid discharge which stains the underclothes.Occasionally faecal matter also stains the clothes and causes more discomfort to the patient.
PAIN:Normally piles are painless unless they are complicated by some secondary problems like fissure,strangulation,thrombosis,ulceration and inflammation etc.
PRURITUS:Perianal skin becomes moist and sodden from irritating mucous discharge,which may occur in the third and fourth degree of prolapsed haemorrhoids.
SECONDARY ANAEMIA: Chronic intermittent blood loss from haemorrhoids may result in severe anaemia.There may be breathlessness,dizziness,severe weakness,palpitation and tachycardia,loss of weight and energy.
